Original Medicare or Medicare Advantage?

Original Medicare and Medicare Advantage are different ways to receive Medicare coverage. A useful comparison starts with the same needs on both sides: the providers you expect to use, covered services, prescription coverage, travel and what happens if your needs change. A low monthly plan premium alone does not answer those questions.

Compare access and coverage conditions

With Original Medicare, you can generally use providers anywhere in the United States who take Medicare. A Medicare Advantage plan may require network providers for non-emergency care and may have referral or prior-authorization requirements. Ask about the particular provider, location and service rather than relying only on an insurer's name.

Original Medicare does not have a general annual out-of-pocket limit without supplemental coverage. Medicare Advantage plans have limits for covered Medicare services, with plan-specific rules. Premiums, drug costs, noncovered services and network differences need their own review. An out-of-pocket limit is not a promise that every health expense falls inside it.

Compare the same question Evidence to obtain for each option
Regular providers Whether the specific provider accepts the coverage
Planned service Coverage, referral and approval requirements
Prescriptions Drug coverage arrangement and the plan's current information
Travel or a move Service-area and emergency/non-emergency rules
Total costs Premiums plus the applicable cost sharing and exclusions
Future change Enrollment windows and any separate Medigap rights

Use a separate row for an unresolved answer. For example, a provider appearing in a directory is a reason to confirm current participation with the plan and provider, not a reason to stop the comparison. Keep the date of each confirmation.

Keep Medigap and drug coverage separate

Medigap supplements Original Medicare; it does not pay Medicare Advantage cost sharing. Most Advantage plans include drug coverage, while Original Medicare can be paired with a separate drug plan. The Medigap rights worksheet explains why the ability to leave an Advantage plan and the ability to buy Medigap are separate questions.

If you have employer, union, FEHB or PSHB coverage, ask that administrator what a change would mean for you and your family before changing anything. Our Medicare learning topic connects the related guides.

Your next step is to compare written coverage information through Medicare, the plans and an independent SHIP counselor if needed. This guide does not rank plans, confirm availability or recommend an individual election.
