Term life
Covers you for a set number of years, such as 10, 20 or 30. It usually costs the least per dollar of coverage, which makes it a popular choice while a mortgage is being paid off or children depend on your income.
Permanent life
Whole life and universal life are designed to last your whole life as long as premiums are paid. Many permanent policies also build cash value you can borrow against, under the policy's terms.
Simplified-issue and guaranteed-issue
Simplified-issue policies ask health questions but skip the medical exam. Guaranteed-issue policies ask no health questions; coverage amounts are usually smaller, and many pay a limited benefit if death occurs in the first years of the policy.
Four life plans, different protections
AFR Life Safeguard
Group term life to age 121, with the listed guaranteed-issue offer up to $50,000 each for eligible employees and spouses. For illness or disease death within the first 12 months, premiums are returned instead of the death benefit, including previously unknown disease. Accident deaths are not affected by that stated limitation.
5Star Life FPPg
Group term life to age 121 with Qualified Issue and Simplified Issue application paths. The listed package does not establish a guaranteed-issue cap. Quality of Life is unavailable at ages 66–70 and on children.
Combined / Chubb Universal Life Protector
Flexible-premium adjustable permanent life. Scheduled premiums may be insufficient to keep the policy in force. The included Accelerated Payment Benefit is described below.
Allstate / American Heritage Life
Term life to age 100. The listed 2021 employee and spouse premiums include the terminal-illness and accidental-death riders below, where eligible. Prices shown are the 2021 biweekly rates. Optional child coverage has a separate premium.

Benefits included in the plans shown
These riders and benefits are included in the selected product packages where eligible. Each plan has its own conditions. See the selected carrier’s child-coverage premium, if applicable.
AFR Life: Life coverage with included living benefits
Better Living can advance 3%–4% of the death benefit monthly, up to 75% total, for the qualifying care needs described in the brochure.
Requires permanent inability to perform at least two of six daily activities without substantial assistance, or permanent severe cognitive impairment requiring substantial supervision. This advances the death benefit; unavailable on children.
AFR Life: Premium relief during total disability
Waiver of Premium is included in the printed adult rates and waives premiums for total disability.
Unavailable after age 60 and unavailable on children. Policy definitions and conditions apply.
Combined / Chubb: Access to life benefits while living
The included Accelerated Payment Benefit can pay partial or full benefits while you are living when its terminal-illness or permanent-confinement conditions are met.
The terminal-illness provision requires diagnosis before age 64 with likely death within 12 months. Alternatively, the brochure covers permanent confinement in a hospital, nursing home, extended-care or hospice facility until death. Policy terms apply.
Allstate / American Heritage Life: Terminal-illness protection included in the shown life premiums
The listed life prices include the Accelerated Death Benefit for Terminal Illness (GTLPLBR). It can advance 75% of the death benefit, up to $100,000.
A physician must diagnose terminal illness. The payment is discounted using the current discount rate; premiums are waived after payment. This is an advance of the death benefit. Prices shown are the 2021 Term to 100 biweekly rates.
Allstate / American Heritage Life: Accidental-death protection included in the shown life premiums
The listed employee and spouse life prices include the Accidental Death Benefit Rider (GTLPADB). The rate sheet lists its benefit at twice the initial death benefit amount.
Printed rider issue ages 18-55. Not available on certificate coverage purchased for a child. Policy accident definitions, limitations and state terms apply. Prices shown are the 2021 Term to 100 biweekly rates.
5Star Life: Quality of Life included for eligible adult coverage
The printed eligible adult rates include the Quality of Life Rider. It advances a portion of the death benefit monthly, up to 75% total, for the stated care needs.
Requires permanent inability to perform at least two of six daily activities without substantial assistance, or permanent severe cognitive impairment requiring substantial supervision. Unavailable at ages 66–70 and on children. This advances the death benefit.
5Star Life: Included terminal-illness benefit
The brochure includes a 30% lump-sum advance for a qualifying terminal condition.
The percentage is 25% in Connecticut and Michigan. Life expectancy must be less than 12 months, or 24 months in Illinois. This advances coverage; policy terms apply.
5Star Life: Included expedited beneficiary payment
The brochure describes expedited payment of 50% of coverage or $10,000, whichever is less.
The stated mailing period is within one business day of notification; death within the two-year contestability period or under investigation is excluded from this expedited provision. This is part of the death benefit, not an additional payout.
AFR Life: Included terminal-illness benefit
The brochure includes a 30% lump-sum advance for a qualifying terminal condition.
Requires a life expectancy of less than 12 months under the printed conditions. This advances coverage; the original certificate terms and first-year illness-death limitation remain.
Eligibility, exclusions, state variations and the original policy or certificate govern. An advance of a death benefit is part of that benefit, not an additional death-benefit payment.
Questions to ask yourself
- How many years would my family need support?
- Is my need temporary, like a mortgage, or lifelong, like final expenses?
- Will the plan need a medical exam or just health questions?
