Medicare coverage notices and appeals

A Medicare bill, coverage notice and explanation of benefits do not all ask you to do the same thing. Before responding, identify who issued the document, which decision it describes and whether it gives a deadline. Keep the complete notice; a summary of the amount alone can omit the instructions you need.

Use the route for your coverage

Original Medicare, Medicare Advantage and Medicare drug plans have different appeal processes. For Original Medicare, start with the Medicare Summary Notice and its appeal instructions. For a Medicare health or drug plan, follow the denial notice and plan materials. Do not copy a deadline from an article about a different type or level of appeal.

A request for a drug that has not yet been covered may begin with a coverage determination or exception request. An exception can require the prescriber's supporting statement. If the plan denies that request, its decision explains the next appeal step. A complaint about service quality is also different from a request to reverse a coverage or payment decision.

Notice detail What to identify before responding
Issuer Original Medicare contractor, health plan or drug plan
Decision The specific service, item, payment or drug being disputed
Process stage Initial request, denial or a later appeal decision
Deadline The date and instructions in this notice
Supporting information What the provider or prescriber can contribute
Submission The official destination and a retained confirmation

Write a brief description of the disagreement in your own files, using the notice's service or decision details. If something is unclear, ask the issuer to explain it while keeping the deadline in view. A telephone conversation should not be assumed to count as a filed appeal unless the process confirms that it does.

Ask promptly about urgent review

Medicare describes fast-appeal rights when covered services are ending too soon. Health and drug plans also have expedited processes in qualifying situations. Ask the provider or plan about the applicable urgent process rather than waiting for a general website response. Eligibility for that process and its deadlines depend on the situation.

SHIP can help explain Medicare rights. A helper may need formal representative authority; merely having a copy of a notice does not establish it. Follow the current instructions for the coverage and appeal level involved.

Your next step is to confirm the route and deadline on the notice and submit through the official channel. Keep Medicare numbers, diagnoses, bills and supporting records private. This guide does not file an appeal, provide medical or legal advice, or predict the outcome.
