AFR guaranteed-issue coverage is shown in the $10,000–$50,000 columns; its first-year illness-death limitation applies. 5Star’s issue paths and Quality of Life limits remain separate. Combined / Chubb scheduled premiums may be insufficient to keep the policy in force; a listed premium does not establish eligibility.
