Coverage questions

Read these source-based explanations with the original policy terms. Eligibility and state availability still apply.

How do 5Star Qualified Issue and Simplified Issue differ?

The Statement of Insurability directs Qualified Issue applicants to answer questions I-III and Simplified Issue applicants to answer all questions. It has separate employee and spouse answers. Your agent will help identify the applicable issue path and coverage limits. Answer the original application questions for the issue path you choose.

Which 5Star ages exclude the Quality of Life rider?

The printed rate sheet excludes Quality of Life at ages 66-70, marked with a star. Quality of Life benefits are also unavailable for children. For eligible coverage, its acceleration is subject to the original permanent daily-activity or severe cognitive-impairment conditions and the 75% maximum.

What child coverage is shown in the 5Star package?

The listed schedule lists $5,000 for $0.46 or $10,000 for $0.92 biweekly. The brochure covers financially dependent children of the employee or spouse from 14 days through 19 years, or 26 if a full-time student. The original age and eligibility definitions apply.

What qualifications accompany 5Star terminal-illness acceleration?

The brochure describes 30% acceleration, or 25% in CT and MI, for the printed life-expectancy condition of less than 12 months, or 24 months in IL. These state and timing qualifications stay with the benefit description. The original policy and certificate determine eligibility.

What does 5Star say about coverage after leaving employment?

The listed brochure says coverage continues after employment terminates, with no loss of benefits or cost increase under that portability wording, after the first premium is paid. It says the insurer bills you directly. Confirm the applicable current certificate and payment requirements before relying on this provision.
