Disability claim and appeal records

A claim file should show what was requested, what was submitted, what the administrator decided and what deadline applies next. Keep those records in chronological order before preparing a response. This checklist helps organize evidence; it does not determine whether a claim should be paid or an appeal will succeed.

Identify the claim and governing arrangement

Start with the actual policy or plan, its edition and the administrator named in the notice. Distinguish a private disability claim from an OPM or SSA application. DOL explains that ERISA generally excludes government-maintained plans, so a private-plan appeal checklist is not a universal procedure for federal benefits.

Ask the administrator or qualified counsel to identify the process applicable to the actual arrangement. Do not rely on a deadline found in a coworker's letter or an unrelated insurer's website.

Create a dated claim index

Record Detail to preserve Question to resolve
Coverage document Policy, certificate, endorsements and effective dates Which version applies to the claimed period?
Initial submission Date, method and receipt What did the administrator receive?
Evidence request Exact items and stated response date Which items remain outstanding?
Supporting record Source, date and secure location Does it address the specific question asked?
Decision notice Stated reasons and cited provisions What facts or terms support the decision?
Appeal instructions Receiving office, method and deadline How will a timely submission be confirmed?
Later determination Result and any remaining route What next step is actually available?

Keep original notices and submission receipts. If the administrator corrects a date or requests a replacement document, preserve both the original and correction so the timeline remains understandable.

Match evidence to the stated issue

Separate questions about coverage in force, the policy definition of disability, the claimed dates, exclusions and the payment calculation. A large pile of medical records may not address a specific missing employer statement or a disputed policy provision.

Ask the administrator to clarify an unclear request. Have qualified professionals address medical or legal questions within their roles. This website does not assess diagnoses, work restrictions or the legal sufficiency of an appeal.

Verify deadlines and submission routes promptly

Record the actual notice's deadline and ask how it is calculated. If a date is unclear or appears close, seek appropriate assistance promptly rather than waiting for a general informational discussion. Do not assume that a request for clarification automatically pauses the deadline.

Use the administrator's confirmed secure route. Keep proof of delivery or submission and ask how acceptance will be acknowledged. A draft saved locally is not a submitted appeal, and a sent message is not proof that every required item was received.

Reconcile payment questions separately

If the dispute concerns an offset, benefit amount or repayment notice, retain the calculation and the period it covers. Use the offset and tax record worksheet to separate gross benefit, deductions and deposits. Do not assume that a payment difference proves either an error or a valid reduction.

Keep sensitive evidence out of general inquiries

Medical records, personnel documents, identifiers and tax forms belong in the appropriate private process. A general inquiry can describe which kind of assistance is needed without attaching the claim file. The useful outcome is a clear record, a confirmed next deadline and the correct responsible office, not a predicted decision.
